We want our campus to be safe and respectful for everyone. If you experience or witness concerning behavior, options for reporting are listed below.

Ways to Report an Incident

1. Report online via the Maxient Reporting Form.
2. Contact the Title IX and TItle 5 Team.
3. Report to the Foothill–De Anza Human Resources Office 
4. Contact Campus Police
   The Foothill–De Anza Police office is located in Room 2103 on the lower level of the Campus Center. Call 408-924-8000 or dial 911 from any campus phone.

Why It’s Important to File a Report

It does not matter when it happened, where it happened, or the surrounding circumstances. By coming forward and reporting, you help make our campus and community safer and help to prevent future harm.

Reporting to the Police — What You Should Know

If you experience sexual violence, you may report to the Foothill–De Anza Police Department. De Anza office is located on the Lower Level of the Hinson Campus Center, across from the Advanced Technology Center. Foothill office is in Room 2103 on the lower level of the Campus Center.

Reporting does not require you to seek prosecution. Police can take steps to help protect you and others.
